How do I create a Teaching Assistant Evaluation survey?
Since TAs are not part of the regular course evaluation system, some instructors or program directors may want to create a TA Evaluation Survey.
Instructors or program directors may create a TA evaluation on an individual course site or may want to create a Project site to administer the TA evaluations.
If a Project site is used, a link to the evaluation may be placed on the course site for students to access the TA evaluation or the URL for the evaluation may be sent to the individual student's e-mail address. In this case, users do not log in to access the evaluation so there is no name associated with any individual evaluation submitted.
If the evaluation is administered through the course site, Instructors (and TAs) will have direct access to the results of the evaluation via the Test & Quizzes tool. However, the results of a survey are usually made anonymous. In this case, instructor (and TAs) can see the results of an individual's survey submission (but may not know the name of the student submitting it).
On the Project site or the Course site, go to Tests & Quizzes.
This displays the Test & Quizzes page.
Note: The Test & Quizzes tool is not a default tool. Yo may have to dd the tol to your tool list using Site Editor / Edit Tools. (See: How do I add or remove tools on my Course/Project site? )
Use the "Select a question type" dropdown box to select a Survey Matrix question type.
Note: You may add any type of question to a survey. if you add a question that requires you to select a "correct" answer, mark any answer as correct and under "Settings" do not grade the survey.

Selecting the Survey Matrix question displays the Edit Survey question page.
Add Instructions (box mislabeled as "question text")
Configure responses and comments.
Configure column widths.
Assign the survey matrix question to a Part of the survey and/or a Question Pool
1. The question can be added to a specific portion of the survey, such as Part 1 or Part 2 (depending on how many parts exist). Part 1 is the default location.
2. The question may also be added to a Question Pool, if desired (See: "What is a Question Pool?")
Provide Feedback. (Optional)
Feedback is optional text available for users to view after the survey is submitted. This feedback will be released to users immediately after submitting the survey if the "Settings" for the survey includes "Feedback upon Submission", "Release Questions and the following" and "Question-level Feedback".(See: "How do I Edit the Settings for an assessment?")
Note: To edit with the Rich Text Editor, click the hyperlink to open the full menu.
Continue adding questions until your TA Evaluation survey is complete.
Click Settings.
After you have added all of the survey questions, click Settings to configure how the assessment will be delivered to the students. For a detailed explanation of the assessment Settings see "How do I configure the Settings for an assessment?"
Click Publish.
Copy the survey URL, then click Publish.
This publishes the survey.
A COPY of the survey is located under the Published Copies tab.
Note: If you selected "Release to Anonymous Users" (for instance, the evaluation is administered on a Project site rather than the course site), the above Publish page displays a URL that you can post on the course site (in Resources or Web Content tool) or you can send the URL to the participants e-mail addresses.
Note: You can retrieve the public URL anytime by clicking Select Actions / Settings (under the Published Copies tab). The URL is now listed under "Assessment Released To".
